Job Description:

Hours: Mon - Fri 8hr shifts between 7am - 4:30pm

Job Summary/Purpose:

The Connections Coordinator will triage phone calls to ensure warm transfer or resolution per department guidelines; provide clerical support between patients, caregivers, physicians, care managers and clinical service departments with routine clerical and administrative tasks; document coordination; and ordering supplies. The Connections Coordinator promotes teamwork, nurtures relationships with others and is viewed positively by co-workers.  Communicates clearly, concisely, and courteously in a timely manner. Demonstrates professional behavior and appearance, respect for patient privacy and confidentiality in all contacts with patients, caregivers, vendors and stakeholders.

Minimum Qualifications:

  • High School diploma or equivalent;
  • Two (2) years customer service experience;
  • Experience with electronic medical records;
  • Knowledge of medical information and terminology
  • Excellent written and oral communication skills;
  • Multi-tasking, flexibility, telephone skills, time management, organization, attention to detail;
  • Proficient typing/keyboard skills required;
  • Strong knowledge of Microsoft Office (Word, Excel, Power Point, Outlook);
  • Some knowledge of medical terminology preferred;
  • Requires a great deal of independent contact with patients and their caregivers;
  • Individuals must have the ability to assess situations where other levels of clinical intervention may be necessary.

Essential Physical Requirements:

  • Lifting up to 25 lbs
  • Job may require frequent bending, twisting, and reaching
  • Ability to sit and/or remain mobile for eight to twelve hours
  • Ability to perform fine manipulation (keyboard)
  • Good visual acuity and acute hearing are required
  • All employees within this job classification may have exposure to blood-borne pathogens, other potentially infectious materials, and/or hazardous chemicals/waste
  • Push/pull up to 200lbs in a wheelchair.
