The Center Nurse Care Manager (Outpatient Clinic Nurse) is a professional caregiver who assumes responsibility for providing patient focused health care in an outpatient setting. The nurse will utilize assessment and critical thinking skills, clear communication, and problem solving techniques to deliver quality care to patients while they are in the Clinic as well as while they are at home.
While the patient is in Clinic, the nurse assesses, plans, implements, and documents care provided to ambulatory patients in an outpatient setting. Nurses assist with minor procedures and provide ordered treatments to patients per physician/practitioner direction. When needed, nurses participate in care conferences to evaluate interdisciplinary interventions and/or patient goal attainment. Nurses provide and document patient education and work cooperatively with members of the Patient Empowered Care integrative teams to provide comprehensive care to patients.
When providing nursing support to patients away from our facility, the Center Nurse Care Manager (Outpatient Clinic Nurse) is a patient advocate and CTCA point of contact responsible for the coordination of all systems/services required for an organized, multidisciplinary team approach. Nurses will collaborate with hometown physicians, insurance companies, specialty pharmacies, home health providers and any supporting agency that supports the care initiated by our physicians for patients. Nurses will speak with patients between visits to CTCA to address concerns that the patient and/or care giver may have to assure continuity of care and optimal clinical and resource outcomes.
- Appropriate state licensure as a Registered Nurse
- BLS certification
- Graduate of NLNAC or CCNE School of Nursing, and licensed in the state of practice
- Bachelor’s Degree in nursing strongly preferred
- OCN Certification within three years of hire
- Current general nursing and oncology experience.
- National certification in specialty population served
- 1-2 years of acute care experience
